Illness & Misadventure Appeal Instructions

1. Email your teacher / KLA Coordinator as soon as you are aware there is a need for an appeal.

2. Download the correct Illness & Misadventure form, as per your year group (see options below)

3. Complete the documentation - including independent evidence from a medical practitioner or other qualified person
   (applicable for Years 9-12).

4. Scan the document
   (the Library Team can help you!)
